KC-AHC—Bridging the Social Needs Gap Between Community and Clinical Providers

Kentucky’s Appalachian region faces a disproportionate burden of unmet social needs compared to the country, which can affect the health of its residents. The Kentucky Consortium of Accountable Health Communities (KC-AHC) helps clinicians identify and address patients’ health-related social needs through routine health checkups and is studying the impact of navigation services on healthcare quality, utilization, and cost.
